    Prices are discussed before evaluation or treatment. When treatment options are discussed, the cost of each will be as well.

    Payments will be made prior to services rendered for evaluations and treatments. Which means no hidden fees or bills after each visit.

  • No insurance

    All payments are made prior to or day of service. No insurance claims will be made on a patient’s behalf. This allows for more time for evaluation and treatment and freedom in developing individualized treatment plans.

  • Office Visit - New Patient, up to 75 minutes- $350
    —Incl: past record review <50pages, evaluation and personalized treatment plan personalized plan.

  • Office Visit - Established, New Problem, up to 60 minutes- $250
    —Incl: evaluation and personalized treatment plan

  • Office Visit - Established, Follow-up, up to 45 minutes- $180
    —Incl: evaluation and personalized treatment plan

  • A non-refundable $50 deposit is required to book a visit, this amount is applied towards the final amount owed.

  • Record Review Plus $5 per 0-10 additional pages

  • Independent Medical Exam, please contact for pricing

  • Sports Physicals, up to 30 minutes- $60
    — Purpose of this visit will be to complete the necessary Pre-Participation Physical Exam (PPE) paperwork. We may discuss eligibility and recommendations on finding for sports participation. This is evaluation only and no treatment of abnormal findings is included.

Notes:
—Injections/procedures same day as Office Visit are a separate charge. Cannot guarantee injection same day as office visit. Any procedure not performed on day of office visit will not require additional office visit charge for procedure only.

—Due to complexity of some problems and extensive medical records, multiple visits may be required in order to obtain a full history and complete exam on each area of concern.

  • Cost of procedures are discussed after thorough evaluation and discussion of options.

  • Prices include ultrasound guidance, supplies, medications, procedural time, facility fees, and post-procedural instructions.

  • Costs of procedures (not incl. PRP) range from $125 to $350. Many factors go into cost including complexity/risk of injection, time needed for injection, facility fees, and cost of supplies.

  • PRP is highly individualized based on diagnosis, area to be injected, individual co-morbidities, and volume need to adequately treat specific area(s). Prices range from $650-$1,250. PRP must be scheduled ahead of time and cannot be added on the same day as an evaluation. Please see PRP page for more information.

CANCELLATION POLICY

  • Must contact to cancel 24 hours prior to appointment.
    Failure to notify clinic of cancellation or re-scheduling prior to this may result in inability to re-schedule or full payment due prior to re-scheduled time. 
    Decisions to re-schedule are at the physician's discretion.

OBTAINING RECORDS

We cannot obtain records from other facilities for you. We will provide guidance on how to obtain records on your own and how to have them sent to us. It is highly recommended that you have printed copies of your medical records and store them in an labeled accordion folder in a three-hole punched binder.

Outgoing records will be sent free of charge in digital formats. Paper copy of records will be printed and sent for $0.25 a page plus cost of postal fees.
