OBTAINING MEDICAL RECORDS

1. DO NOT request records be sent by print in the mail.

2. Request records for pertinent specialists/specialties including clinic notes, surgical reports, procedures reports, imaging reports for Xray, CT, MRI, and Ultrasound, etc.

3. Request records dating back for as long as you think you may have been seen for the issue you are being seen for. Minimum is usually 3 years.

4. If you cannot remember the name of the specialist you saw, listing the specialty is usually fine or just saying all records during a specific time frame.

5. Have records sent by e-mail or fax.

6. Imaging files at most Omaha area clinics/hospitals can be shared with us through:
   Nuance Powershare. You may need to indicate this on your form.
